Fountain Social Care Network, operated by Redeem House of God Church Inc., is seeking compassionate, dependable, and community-minded professionals who want to help individuals and families connect with essential health and social care resources, services, and supportive programs.

Our team works with individuals and families experiencing health-related social needs, including housing instability, transportation barriers, difficulty accessing community resources, and other challenges that can affect health, well-being, and long-term stability.

Available Positions

Social Care Navigator

Pay: $24.50–$25.00 per hour

Social Care Navigators work directly with members to identify needs, provide personalized navigation, connect them with appropriate services, and complete timely follow-up to support successful access to care.

Responsibilities may include:

HRSN screening and navigation support

Connecting members with trusted community resources

Housing and benefits referrals

Member follow-up and ongoing support

Care coordination with service partners

Accurate and timely case documentation

Communication with community providers and agencies

 

 

HRSN Screening & Navigation Specialist

Pay: $24.50–$25.00 per hour

The HRSN Screening & Navigation Specialist assists members with Health-Related Social Needs screenings and helps eligible individuals understand their options, connect with appropriate services, and access available community resources.

Responsibilities may include:

Completing HRSN screenings accurately

Identifying immediate and ongoing social care needs

Providing compassionate navigation services

Coordinating referrals with community partners

Following up with members as needed

Maintaining accurate and complete documentation

Housing Support Specialist

Pay: $24.50–$27.00 per hour

The Housing Support Specialist works with individuals experiencing homelessness or housing instability by providing housing navigation, stabilization support, resource coordination, and guidance toward safe, sustainable housing options.

Responsibilities may include:

Housing navigation and resource education

Housing stabilization planning and support

Landlord and community resource coordination

Housing applications and referral assistance

Permanent housing referrals and follow-up

Applicants should demonstrate professionalism, compassion, confidentiality, dependability, strong communication skills, accurate documentation, cultural awareness, and respect for the individuals and families we serve.

Specific education, experience, training, driver's license, background screening, and other requirements may vary by position.

Employment Information

Positions may be available on a full-time, part-time, or per diem basis, depending on program needs, funding, and service volume.

Compensation may vary based on experience, qualifications, responsibilities, and position requirements.
