Fountain Social Care Network (SCN)

Join Our Growing Social Care Team

Fountain Social Care Network, operated by Redeem House of God Church Inc., seeks compassionate, dependable professionals dedicated to connecting individuals and families with essential health, social care, and community resources.

We support people experiencing health-related social needs, including housing instability, transportation barriers, limited access to resources, and other challenges affecting health, well-being, and stability.

Social Care Navigator

Pay: $24.50–$25.00 per hour

Social Care Navigators identify member needs, provide personalized navigation, connect members with services, and follow up to support access to care.

Responsibilities may include HRSN screening, resource and benefits referrals, care coordination, member follow-up, provider communication, and timely case documentation.

 

HRSN Screening & Navigation Specialist

Pay: $24.50–$25.00 per hour

The HRSN Screening & Navigation Specialist completes Health-Related Social Needs screenings and helps eligible members understand options, connect with services, and access community resources.

Responsibilities may include accurate screenings, identifying immediate and ongoing needs, compassionate navigation, community referrals, follow-up, and complete documentation.

Housing Support Specialist

Pay: $24.50–$27.00 per hour

The Housing Support Specialist assists individuals experiencing homelessness or housing instability with housing navigation, stabilization support, resource coordination, and guidance toward safe, sustainable housing.

Responsibilities may include housing navigation, stabilization planning, landlord and community coordination, application assistance, referrals, and follow-up.

 

RHOG SCN Internship Program

Redeem House of God Church Inc. – Social Care Network (RHOG SCN) offers supervised, hands-on internships for students and emerging professionals interested in social care, human services, healthcare administration, outreach, housing services, and care coordination.

Interns gain practical experience in a community-based organization serving individuals and families with health-related social needs.
