The Fountain Social Care Network, operated through Redeem House of God Church Inc. in Syracuse, takes a whole-person approach to help individuals and families overcome barriers to health, housing, and everyday well-being.
Our core network services
We do far more than simply provide a phone number or referral. We conduct health-related social needs (HRSN) screenings, assist eligible Medicaid members, and actively walk alongside individuals to navigate practical resources for long-term independence.
Care navigation and screenings
We provide comprehensive HRSN screenings and hands-on guidance to connect you with vital community resources, benefits, and healthcare programs.
Housing and stabilization
Dedicated transitional housing and stabilization support for people experiencing homelessness, leaving treatment, or seeking safe recovery environments.
Food and transportation aid
Nutritional pantry assistance, emergency food supplies, and reliable non-medical transportation to ensure you can reach critical appointments.

Our network is open to individuals and families across Syracuse and neighboring areas facing complex challenges. Whether you are facing housing loss, transitioning out of a shelter or recovery center, returning to the community after incarceration, or coping with food insecurity, our team is here to listen without judgment and meet you exactly where you are.
